Event Reservation and Membership Form

To register for an event or to join the PWS, print our Event Reservation and Membership Form, complete it, and send the completed form and your check, to:

Events that are currently available for reservations are listed in the bottom of the form. Reservations will be accepted for Sold Out events until there are at least 5 people on the Waiting List. To register for an event, list the attendees, indicate the number at each level, and enter the event total.

To join PWS or to renew your annual membership, complete the top part of our Event Reservation and Membership Form. Circle the desired membership level and enter the dues total.
